Pass along the knowledge that does not fit in a checklist
A caregiver learns countless details that never appear in a medical chart: what is normal, what a certain expression means, which abilities should be encouraged, and what may feel frightening or disrespectful. Those details can transform a handoff.
This two-page letter-style printable gives the family caregiver space to explain who the person is, what makes a difference, how they show happiness or discomfort, what they can do for themselves, what worries the caregiver, and whom to call with questions.
Write what another person cannot easily observe
- Use plain, specific language and a few real examples rather than trying to document everything.
- Name strengths and abilities so helpers know where to allow time and independence.
- Include dignity and privacy needs, then review the page with the person you support whenever possible.
