What the EMS refrigerator sheet is for
During an emergency, responders may need a clear snapshot before anyone has time to find and explain a full set of records. This one-page sheet keeps essential information together: identity, allergies, selected health conditions, key medicines, communication needs, emergency contacts, and where the fuller caregiver binder is kept.
The sheet is designed for the refrigerator because many fire departments teach responders to look there. Local practices differ, so ask your fire department whether it participates in a Vial of Life, File of Life, or similar emergency-information program.
How to prepare and place it
- Fill in current information and add a recent photo when appropriate.
- Write down exactly where the fuller caregiver binder and any signed advance directive, POLST, or DNR form can be found.
- Place the sheet in a visible location on the refrigerator and tell trusted helpers it is there.
- Add the date each time you review it, and replace the sheet whenever information changes.
EMS can generally follow only a signed directive form they can see, and requirements vary by state. Follow local guidance and advice from qualified professionals. In an emergency, call 911 or your local emergency number.
