Bring the right information into a stressful visit
Emergency departments move quickly, and caregivers may be asked to explain what changed while also finding medicines, documents, and comfort items. Preparing those details before a crisis creates a calmer starting point.
The first page gathers the person’s usual baseline, recent changes, dementia or communication needs, allergies, medicines, legal-document locations, and the person authorized to speak with clinicians. The second page turns discharge planning into a clear checklist.
How to prepare the go-sheet
- Complete the baseline and contact sections now, before an emergency happens.
- Keep the sheet at the front of the binder and note where the go-bag and legal documents are stored.
- At discharge, write down medicine changes, follow-up appointments, warning signs, equipment, and whom to call with questions.
