Make shared care specific enough to be dependable
People often want to help but do not know what would be useful. Caregivers may also carry the entire schedule in their heads, making a last-minute cancellation harder to solve. A written plan makes the work visible and the asks concrete.
The printable includes weekly coverage by time of day, a rides-and-errands list, a place to record offers of help, and a backup plan naming the first calls, agency options, and where the full handoff information is kept.
Turn “let me know” into one clear task
- Write a name in each coverage block only after the person confirms.
- Record specific tasks such as a pharmacy pickup, grocery run, appointment ride, or two-hour visit.
- Identify the first and second backup calls and make sure they know where the daily routine and binder are stored.
This sheet coordinates help; it does not replace the care plan or professional instructions.
