Connect appointment planning with what happened afterward
Caregivers often track appointments in one place and visit instructions somewhere else. This log keeps the practical plan and the outcome together so follow-up tasks are less likely to disappear after a busy visit.
The first section records dates, clinicians, purpose, address, transportation, and required preparation. Repeatable visit-note blocks capture findings, medicine changes, tests, next appointments, and questions for next time.
Use the log before and after every visit
- Add the appointment as soon as it is scheduled, including who is going and any fasting or preparation instructions.
- Bring the Doctor Visit Prep Sheet and current Medication & Allergy List.
- Before leaving, ask for the after-visit summary and record medicine changes, tests, follow-up, and unanswered questions.
Follow the care team’s instructions. When you are unsure about a change, call the clinician’s office.
